How to Build a Signature Style Agreement in Forms

  1. Create a new form in Coach Catalyst.

  2. Add a Section Header.

    • Name the header after your agreement or waiver.

      Example:

      • Liability Waiver

      • Membership Agreement

      • Policy Acknowledgment

  3. Paste the full text of your agreement into the description area under that header.

    • This is where all terms, policies, and explanations go.

  4. Add a Question at the end of the form.

    • Use a text based question.

    • Title it Signature.

    • In the description, write something like:

      • By typing your name below, you acknowledge that you have read and agree to the terms of this agreement.

      • Make sure to make this signature question "Required"

  5. Save and send the form to your clients.

This method automatically records a timestamp when a client completes the form.

For many regions and use cases, this satisfies common legal requirements for acknowledgment.
