We have created some done for you forms to get you started.

When you save these Programs with the Forms in them, if you want the forms to be attached to the metrics in your account, you will need to attach the metrics.

Here's how:

  1. Make sure you have the Metrics created (How to create a metric)

  2. Open the Form you want to connect to the metrics

  3. Click the gear to the right of the metric

  4. Turn on the Connect to Metric Option

  5. Select the Metric to connect to

  6. Save Form
